  • Lab work and consults will be automatically billed every five (5) months, and it is my responsibility to complete them on time to ensure medications can be dispensed and shipped without interruption.
  • I understand that all medications and treatment protocols require lab work and a consultation with one of 99 TRT Club’s licensed clinicians, and approval prior to medications being shipped.
